Updating Beneficiaries

Beneficiaries can be updated at any time. To update your beneficiaries, you must submit the following forms to the Employee and Retiree Service Center. Please be sure to save a copy of all forms for your records.

Life Insurance

Basic Employee Life Insurance 

•  MCPS Form 455-20: Employee Benefit Plan Enrollment, Section VI

Optional Employee Life Insurance 

• MCPS Form 450-1: Optional Employee Term Life Insurance Enrollment 

Basic Dependent Life Insurance and Optional Dependent Life Insurance 

• You are always the beneficiary for this coverage

Update Retirement System Beneficiary: State/County

State Core Pension Plan Members 

• State Form MSR004  
• MCPS Form 455-5: Designation of Beneficiary/Beneficiaries 

MCPS Core Pension Plan Members 

• MCPS Form 455-5: Designation of Beneficiary/Beneficiaries 

Update Defined Contribution Plan Beneficiary [403(b)/457(b)]
