There are two ways you can make necessary updates to this form:
Go to ParentVue, Click on Online Registration at the top of the page, click on Annual Verification 2025-2026, make any necessary updates to the information, electronically sign and hit submit.
Make any necessary changes to the form directly on the form. Please sign the back of the form and send it back to Candlewood with your student.
If there are no changes to the information, you do not need to go to ParentVue or send back the form.
