MCPS encourages parents to enroll their students at their home school’s Kindergarten Orientation event. 

 

Woodlin’s Kindergarten Orientation event will take place on April 23 and April 24, 2020.   

 

To reserve a day and time to enroll your child for Woodlin’s Kindergarten Orientation, please contact the school’s Kindergarten Enrollment Coordinator, Mr. Hazel, by email or by phone.

At Orientation, you can bring your child to the school to meet the kindergarten teachers and participate in some activities while you are completing the enrollment process.

 

The list of the required documentation and forms for kindergarten enrollment is as follows:

 

1.  Original birth certificate

2.  Proof of residency, which is your deed if a newly purchased house or your Montgomery County property tax bill (not the tax assessment) if you have been living in the residence long enough to receive your tax bill, or a lease for renting.  If you are in a shared housing situation, you would need to complete a shared housing form.  You can find that form on the MCPS website.  Search for “Shared Housing Form” in the search line.  In addition, please provide three (3) pieces of regularly delivered mail with the parents name and home address on it.  These can be a utility bill, mobile bill, insurance bill, bank statement, etc. 

3.  Social security card/number for the child

4.  Picture ID of registering parent (for example, a driver’s license)

5.  MDH Form 896, Immunization Certification; to be completed by an authorized health care provider.  The school must have this form prior to the student attending school.

6.  Form 565-1, Student Emergency Information

7.  Form 560-24, New Student Information

8.  Form 345-17, Prekindergarten Experience

9.  Form 525-17, Dental Health Form (to be completed by an authorized health care provider)

10.  Form SR-6, Health Inventory/Record of Physical Examination Form (to be completed by an authorized health care provider)

11.  Form 4620, Blood Lead Testing Certificate (to be completed by an authorized health care provider)

 

You can find all the forms on the MCPS website.  Just enter “Form” with its number into the Search Line.  You will need to print and complete these and bring them to the school.
