Consent Forms for Student Data Questionnaire
On Tuesday, April 9, 2019 Montgomery County Public Schools (MCPS) will offer the SAT free of charge to all Grade 11 students at Northwood High School. If your child signed up to take the school administration of the SAT, they will have the opportunity to participate, with your consent, in the optional Student Data Questionnaire. The College Board (the provider of the SAT), gives students and families complete discretion regarding any voluntary information they provide as part of the Student Data Questionnaire. To provide consent for your child to participate in the optional Student Data Questionnaire, you must return a signed consent form before the school pre-administration session. Please find the consent form, which provided more detailed information, and the school guide about how to fill out the form via the MyMCPS portal in the attachments. Please return either this form to the main office or complete the form via the MyMCPS portal by Friday, March 22. Email any questions to Holly Jones, Academy Coordinator, at Holly_M_Jones@mcpsmd.org.
