    Registration for the upcoming middle school athletics fall season for 7th & 8th grade students: (softball and cross-country) will be submitted through the ParentVue Portal (currently open).

    Students should complete their Physical Form so that the medical eligibility form (last page of SR-8) is ready for online submission via the ParentVue Portal, prior to tryouts/practices. A hard copy of the form may also be handed in to Mr. Seidman.

    This form is valid for two years from the date of the exam and covers participation for all sports within that time period. This form must be completed and/or be on file before a student-athlete can participate.

    Important Forms:

    All three of these forms must be filled out and handed to Mr. Seidman by the first day of try-outs.  Any student that does not have all three forms filled out will not be allowed to participate.

    • Students may not participate unless all forms are turned in to your coaches.
