    The Eastern Middle School Health Room is staffed by Jingle Aben, BSN, RNC-LRNN, School Community Health Nurse, and Irene Olisa, School Health Room Technician.

    Health Forms for New Students

    Students enrolling in MCPS must provide updated immunization records and the following health forms:

    • Student Record Card 6 (SR-6): Complete Part I; the student's health care provider completes Part II. Download Form

    • Dental Health Form: Complete Section I; the student's dentist completes Section II. Download Form

    Incoming 7th Grade Students

    Maryland regulations require all 7th grade students to provide documentation of the following vaccinations:

    • Tdap (tetanus, diphtheria, and pertussis)

    • MCV (meningococcal)

    Please provide updated vaccination records to the Health Room as soon as possible.

    Medications at School

    A medication authorization form is required for all prescription and over-the-counter medications administered at school.

    The form must be signed by both a parent/guardian and the student's health care provider. All medications must be brought to school by an adult in the original, labeled container along with the completed form.

    Immunizations

    All students must meet Maryland's required school immunization requirements. Families will be contacted by the Health Room if additional documentation is needed.

    When to Keep Your Child Home

    Please keep your child home if they have:

    • A fever over 100.5°F

    • Active vomiting

    • Diarrhea

    Please do not send a student to school after giving medication such as acetaminophen or ibuprofen to mask a fever or other illness.

    Please notify the Health Room of any changes in your student's health condition. A physician's note is required after a student has been absent for three days.

    Make sure your emergency contact information is current with the school.
