Maryland Medicaid and Medicaid Waiver Programs
- Maryland Medicaid
- Rare and Expensive Case Management (REM) Program - The Rare and Expensive Case Management (REM) Program is a case managed fee for service alternative to HealthChoice Managed Care Organization (MCO) participation for recipients with specified rare and expensive conditions.
- The Model Waiver Program - The Model Waiver allows medically fragile individuals who would otherwise be hospitalized and are certified as needing either hospital or nursing facility level of care to receive medically necessary and appropriate services in the community. As a result, the Model Waiver clients can continue to live at home with their families.
- Community First Choice, Community Options, Community Personal Assistance and Increased Community Services - The programs provide assistance with activities of daily living to Medicaid recipients who have a chronic illness, medical condition or disability. Services are provided in the eligible individual's home or community residence. Each program has different requirements based on financial eligibility and level of need.

- Maryland Family Network: Child Care - LOCATE: Child Care can help you find service available to parents made possible through funding from the Maryland State Department of Education. LOCATE has a complete database of all regulated child care in Maryland. Your LOCATE special needs child care referral specialist will provide you with supportive information.

- ChildLink - (240) 777-4769 - providing parents and caregivers with resources and information about services and linkages to programs that are available to assist them with children birth to age five.
State
- Division of Rehabilitation Services (DORS) - (301) 949-3750 - The Department of Rehabilitation Services provides short term funding to programs for individuals with disabilities found eligible to meet employment and independence goals. Provision of services is based on funding availability.
- State of Maryland Developmental Disabilities Administration (DDA) - (301) 362-5100 - The Developmental Disabilities Administration provides long term funding for individuals eligible for adult day, residential, or support services for individuals with developmental disabilities to work and live as independently as possible in the community. Eligibility is based on priority, availability of needed services and funding, and acceptance by the agency of the client’s application.
- DDA's Low Intensity Support Services (LISS) Funds: Provides up to $2000 to assist eligible children and adults with developmental disabilities with purchasing eligible services and/or items to address their needs.

- MyTurn - Montgomery County residents ages 3 to 13 years of age with a developmental disability. Provides short term or one time only financial assistance to caregivers of children with developmental disabilities and assists caregivers with accessing information, linking to other community resources and designing individual support services.
