Reporting Fatalities and Severe Injuries

Maryland Occupational Safety and Health (MOSH) requires employers to report work-related fatalities and severe injuries. Work-related fatalities (which occur within 30 days of the occurence of an injury or illness) must be reported to MOSH within 8 hours; work-related inpatient hospitalizations, amputations, and losses of an eye (which occur within 24 hours of an injury or illness) must be reported within 24 hours.

MCPS school principals and facility administrators must contact MCPS Systemwide Safety Programs immediately (301-370-2141) if any work-related fatatalities or serious injuries/illnesses occur at their facilities. MCPS Systemwide Safety Programs will report these incidents to MOSH and conduct an investigation.

OSHA Employee Injury/Illness Logs

MOSH requires employers to maintain records at each work site of occupational injuries and illnesses on the Occupational Safety and Health Administration (OSHA) Form 300: Log of Occupational Injuries/Illnesses. A preprinted form, OSHA Form 300A: Summary of Work-Related Injuries and Illnesses, summarizing recordable occupational injuries and illnesses at each school/facility during the previous calendar year must be posted by each school/facility from February 1 through April 30. These forms are distributed, via Pony, to all MCPS schools/facilities each January by MCPS Systemwide Safety Programs.

All schools/facilities must maintain their OSHA 300 logs for five years following the end of the associated calendar year. Logs must be available at school, department, or division offices for inspection by MOSH representatives and the MCPS Systemwide Safety Programs team leader. Current and former employees and their authorized union representatives may also request a copy of their facility's OSHA 300 log, which must be provided by the end of the next business day.


OSHA 300, Log of Work-Related Injuries and Illnesses

Each January, all MCPS school/facility principals and administrators will receive, via Pony, their file copy of OSHA Form 300 that lists names of employees with recordable injuries and illnesses for the previous calendar year. These logs must be maintained and retained for five years following the end of the associated calendar year. Logs must be available at school, department, or division offices for inspection by MOSH representatives and the MCPS Systemwide Safety Programs team leader.The OSHA 300 log must not be posted.


OSHA 300A, Summary of Work-Related Injuries and Illnesses

Each January, all MCPS school/facility principals and administrators will also receive, via Pony, their OSHA Form 300A that shows the total number of recordable incidents for their facility. Names and injury/illness descriptions for individual cases are not provided on this form. This form must be posted on a staff bulletin board or a similar area accessible to all staff members from February 1 through April 30, as required by state law.
