PTA Forms

Cash Receipts Log (PDF): The Cash Receipts Log should be used when goods are sold (tickets, soda, candy, etc.) at an event. The log tracks goods sold and money collected. This form must be given to the treasurer or other BGES Board Member at the end of an event.
  
Expense/Reimbursement Form (PDF): The Expense Reimbursement Form: should be used for parents to get reimbursed by the BGES PTA for expenses they incurred. Receipts must be attached and the form must be signed by a PTA Board Member.
