           Mid-Year Request Form            
*   Authorization for Release of Records(needed only once)
                           Request for Transcripts (1 for every college)        
       *  Secondary School Report form  (needed only once)
         *   B-CC High School Reflections Sheet (Self Assessment)

    HOW TO MAKE AN APPOINTMENT WITH YOUR COUNSELOR:

Students can come to the Counseling Office to see their counselors on a walk-in basis before school, during lunch or after school. They can make appointments to see their counselors during the school day with Ms.Goltz.
