Student Medication Authorization Forms

PARENTS NEED TO BRING ALL MEDICATION TO THE SCHOOL AND CONFIRM THAT ALL FORMS ARE FILLED OUT PROPERLY BEFORE MEDICATION WILL BE ADMINISTERED AT SCHOOL!!!!!

Over the counter medication must be unopened .

Anaphylaxis

We need Medication Form and Allergy Letter Filled out Yearly and Submitted to the Health Room.

 Asthma 

We need Medication Formand Asthma Letter Filled out Yearly and Submitted to the Health Room.

Seizure 

We need Medication Form and Seizures Letter Filled out Yearly and Submitted to the Health Room.

 Medication/ Treatment 

If you students need Medication or Treatments done at school, please fill out proper forms yearly.
