ATTENTION: FREE Seasonal FluMist Vaccine Clinic at Clearspring
We will be holding a FREE FluMist Vaccine Clinic at Clearspring Elementary School on November 11, 2011 from 1:00pm – 3:00pm.
Please call the health room at 301-253-7098 or email Gail_Powell@mcpsmd.org for an appointment.
Notification letters, consent forms, and Vaccine Information Statements in both English and Spanish are below.
Remember: Children must be accompanied by an adult who will bring the completed consent form to the clinic. Please do not send consent forms back to school.
