Helpful Forms for Parents/Guardians - Below are forms that may be useful during the school year. There forms are optional and are provided for your convenience.
- Student Release Authorization Form - This may be used to give permission to release your child to individuals other than the parent/guardian. It will be used in addition to the Student Emergency Form.
- A Note for Belmont - This may be used to notify the teacher and Main Office of absences, tardiness, leaving early and change in dismissal. A Note for Belmont - 2 per page
- After-School Activity Blanket Permission Form - This form may be used to give permission for your child to attend an after-school activity at Belmont for the entirety of the program.
Attendance and dismissal plans - When notifying your child's teacher by email about attendance and/or afterschool/dismissal plans, please include our Main Office Staff in the event the teacher has a substitute teacher: Theresa_Collins@mcpsmd.org
Harris Teeter Education Program - Belmont Elementary proudly participates with the Harris Teeter Together In Education (TIE) to help raise funds for our school. This program is free and does not interfere with your VIC savings. Simply link your Harris Teeter VIC card to our school or schools or choice. The code for Belmont ES is: 1115. Giant A+ School Rewards Program - Unfortunately, this program has been discontinued by Giant and we will no longer be able to collect A+ Bucks Rewards.
