Effective August 26, 2019, this report may be done online after making the initial phone call report to Child Protective Services at 240-777-4417. As of that date, the person making the oral report may now submit the written form online within 48 hours. The form will automatically be sent to each of the four necessary locations noted on the paper version of the form confidentially, and the reporter will no longer need to submit hard copies to each of the required offices noted below. The option to complete the paper Form 335-44 is still available if the reporter chooses to submit the reports through the existing forma; however, MCPS is strongly encouraging this online format, rather than paper forms, as this accelerates and streamlines responses from our partner agencies.
INSTRUCTIONS: An oral report must be made immediately to Child Welfare Services, Montgomery County Department of Health and Human Services (DHHS), (commonly known as Child Protective Services (CPS)), at 240-777-4417. For vulnerable adults, call Adult Protective Services (APS), at 240-777-3000. If this is an emergency, call the Montgomery County Police Department (MCPD), at 911 or 301-279-8000 immediately.
Complete and submit a written report, via the online form (preferred) or the paper form below, within 48 hours.

MCPS employees, contractors, or volunteers may not interrogate or collect written statements from any alleged victim, witness, or offender.
