GENERAL INSTRUCTIONS: This form is used by central office and school administrators to request approval for an employee to serve in a higher level assignment (HLA) to fill a vacancy due to long term absence or position lapse. All requests for higher level assignments must be made in advance.
All higher level assignments will end on June 30 of the fiscal year. If the need for a higher level assignment crosses fiscal years, two separate requests should be submitted, one for each fiscal year.
