WJ Athletic Department-Required Paperwork

The following paperwork is required for athletes on the first day of tryouts/practice.
All documents may be printed from the links above or picked-up in the WJ Main Office.
Printing the forms from the website requires the Adobe Acrobat Reader.
The County Activity (ECA) Fee is not payable until students make a team or
participate in another school extra-curricular activity. The annual fee for 2011-2012 is $30.
