Required Paperwork
All of the following required forms are available through the school or here online and will need to be completed in order to participate in the Wheaton High School Sports Program. All forms must be completed before an athlete can participate. Come and be ready to play. Be sure to get your physical completed, and bring the physical form and signed parent permission form for the first day of practice.
- Health Form -A new physical exam must be done each year.
- Athletes Medical Card - This form must be completely filled out and signed by a parent/guardian before a student may participate in any of the sports programs. Physicals will be done in the Health Room, dates and times will be announced in advance. Bring the completed Heath Inventory Form and $25 cash.
