Please contact the counseling office at 301.320.6525 to set up an appointment for enrollment. At the time of enrollment, you will need to provide the following verification documentation:

Proof of age – examples include:

Birth certificate
Passport/Visa
Hospital Certificate
Other legal or notarized identification

Identity of person enrolling student and relationship to student – examples include:

Photo ID
Driver’s License
Passport
Permanent Resident Alien Card
Court Order

Residency information – examples include:

Lease (less than 1 year old)
Settlement papers
Property tax bill
Shared Housing Disclosure form and supporting paperwork (http://www.montgomeryschoolsmd.org/departments/forms/pdf/335-74.pdf)
