If you are planning to enroll your
student at Takoma Park Middle School, there are
certain documents you must both bring with you and
complete before your student can attend school. The
lists below are provided to help make the registration
process as easy as possible. Any questions regarding
registration should be directed to the Guidance Office
and Ms. Marci Ponton, our registrar. Please call for

Your current property tax bill or
settlement sheet if you own or your lease if you
rent. Older documents must be accompanied by a
current utility bill (electric, gas, or water) in
the parent or legal guardian's name. If the
person enrolling the student is not his/her parent,
the family is referred to the Residency and Tuition
Office (301) 929-2025.

Student's birth certificate. Students
not United States citizens must call the
International Student Admission Office at (301)
230-0686 to begin the enrollment process.

Name and mailing address of last
school attended by student

Most recent report card

Student transfer card if student is
coming from a Maryland or Washington, DC public
school

A copy of the IEP if student
receives special education services
The parent or legal guardian of the
student must complete and sign the following:

Registration Form for the designated grade level.
This form is on the last page of each grade level's
registration packet available through the counseling
office.
Please note that new students enrolling
in July and August are also required to complete the summer
reading assignment.
If the student wants to participate in
any extracurricular activities, the Extracurricular
Activity Form must be completed and the fee paid.
Remittance and mailing instructions are on the bottom of
the form.
