Athletics Try-Out Forms

The following forms are due on or before the first day of tryouts to Ms. Woodward. You will not be allowed to try out without having all forms completed and turned in.

Parent/Guardian Permission Form

Completed form must be signed and dated by parent/guardian.

Health inventory (MCPS Form SRS-6)

If you already have a medical on file, get a note from the nurse stating your medical is good through the end of the particular season. Physicals are good for two years.

Medical Card for Athlete (MCPS Form 560-30)

Completed form must be signed and dated by parent/guardian.

Application to Participate in an Activity Away from School (MCPS Form 560-31)

Player should complete the first line in Part I. Part III should be signed by parent/guardian.
