Required Forms for Participation in Athletics

Prior to participation in try-outs or practices with a team,
the student-athlete must supply to the coach the following:

A current medical evaluation (good for exactly one calendar year) completed on the Health Inventory Form, MCPS Form SRS-6.
The form is to be properly completed by a physician/nurse practitioner.
A signed Student-Parent Athletic Participation Contract and Parent Permission Form, which can be found in the Student-Athlete Handbook. Parents and students are required to read the handbook prior to signing
the above mentioned form.
Completed Medical Card for Athlete, MCPS Form 560-30.

Prior to participation in a contest,
the student-athlete & parent/guardian must complete the following:

Made payment to the county for the Extracurricular Activity (ECA) Fee.
Completed Application to Participate in an Activity Away from School for which MCPS Transportation is Not Provided, MCPS Form 560-31, if student-athletic takes any form of transportation other than buses provided by the school. This form will be handed out by the coach of each team.
