INFORMATION FROM YOUR PUPIL PERSONNEL WORKER

Change of School Assignment (COSA)

Apply from February 1 – April 1, 2008 for the 08-09 School Year

Do you think you have a circumstance which requires your child attend a school other than their home school? If so, Montgomery County Public Schools Pupil Personnel Workers (PPWs) are here to help you understand the process.

Students are expected to attend their home schools but in certain circumstances, a change of school assignment may be approved.

The Change of School Assignment (COSA) form is the official request. Both the form and additional information are available online at:

www.montgomeryschoolsmd.org/info/transfer.shtm (MCPS Form 335-45). These documents are also available in every Montgomery County public school.

If you decide to submit a request, here are some helpful hints:

Thoroughly read through the requirements, information and directions.

 You should also know that:

-once your form is complete, you should take it to your home school for the principal’s signature and transmittal to the appropriate office.

-you may be contacted and requested to supply additional information.

-you will not receive an immediate answer. The process may take up to two weeks, so please be patient. The decision will be mailed to you in letter form.

If your child receives more than 15 hours of special education services, a request for a change of school assignment request should go in letter form to: Department of special Education, Division of School-based Special Education Services.

For more information about the transfer process, contact the principal of your child’s home school.
