Download the 'Official" Immunization Certification Form for your pediatrition to complete (not due to the preschool until orientation in October, but don't wait too long for to make your doctor appointment!)

Choose one of the methods below to submit your application.
1. You may email the form back to the lab school (click here) as a file attachment. Be sure to send in your one hundred dollar deposit to the address below. Please make checks payable to: QOHS.
2. Print the application and mail the completed form along with your one hundred dollar deposit to:
