PTSA Income Submission/Request for Payment Form [Replaces Cash Verification Form and Cash Disbursement Form]
Income: Section A and B of this form are required when submitting money for deposit. All monies collected by PTSA representatives must be submitted directly to the Treasurer or designated representative.
Payment: Section A and C of this form are required when requesting payment. All expenses must be approved by the Board before being incurred and must be in compliance with the approved Operating Budget.
