Outdoor Education

Students and guardians should receive the packet, in the mail, the week of November 2nd.

On Monday, Wednesday, and Friday mornings at the 6th grade entrance, an adult will be collecting the following:

Payments,
Outdoor Ed. Program,
Parent Permission slips,
Chaperone Forms,
Disclosure Forms,
Medication forms,
Written notes to Ms. Abreu

Parents and/or guardians please keep a lookout for the guardian/student Outdoor Education packet coming to you the last week of October.

The “Physician Order Form” is required for ALL meds accompanying your child on Outdoor Education.

If your child has medication in the Health Room already, it will be sent to Outdoor Education.  No further action is required. 

Prescription medications- the parents fill out the top, the doctor is required to fill out the middle portion of the medication order form.  Medical forms in both English and Spanish are available in the main office.  The English form must be filled out, the Spanish form may be used as a reference.  

For over-the-counter medications-the entire  form can be filled out by the parent-no signature is required from the doctor

**NO MEDICATION WILL BE ALLOWED ON OUTDOOR EDUCATION WITHOUT FORM FILLED OUT APPROPRIATELY.  Please submit the forms and medications to the Health Room as soon as possible.
