Required paperwork to participate in Kennedy Athletics

All of the following forms are available through the school or here online.  All forms must be completed before an athlete can participate.

  • Medical Exam (MCPS SRS-6 or equivalent) - Note: A new physical exam must be done each year.
  • Medical Card (MCPS 560-30) - This form must be completely filled out and signed by a parent/guardian before a student may participate.
  • Team Standards Form - This form must be completely filled out and signed by a parent/guardian.
