Required paperwork to participate in Kennedy Athletics

All of the following forms are available through the school or here online. All forms must be completed before an athlete can participate.

Medical Exam (MCPS SRS-6 or equivalent) - Note: A new physical exam must be done each year.
Medical Card (MCPS 560-30) - This form must be completely filled out and signed by a parent/guardian before a student may participate.
JFK Athletics Contract- This form must be completely filled out and signed by both athlete and a parent/guardian.
