Required Paperwork
· A current medical evaluation (good for exactly one calendar year) completed on Health Inventory Form, MCPS Form SRS-6 . The form is to be properly completed by a physician/nurse practitioner.
· Signed Parent/Guardian Permission Form with written acknowledgment of insurance coverage.
· Completed Medical Card for Athlete, MCPS Form 560-30 . Required Paperwork
