Required Forms for Participation in Athletics

If you have questions regarding a particular sport, please contact the coach. Some sports have further information on this website; on the main athletic page, just click the name of the sport to follow the link.

In order to participate in an interscholastic sport each candidate must have the following completed:

• A current medical evaluation (good for exactly one calendar year) completed on Health Inventory Form, MCPS Form SRS-6 .  The form is to be properly completed by a physician/nurse practitioner.

• Signed Parent/Guardian Permission Form with written acknowledgment of insurance coverage.

• Completed Medical Card for Athlete, MCPS Form 560-30 .

• Have paid the MCPS Extra curriclular fee which you can now do online.

Sometimes an Application to Participate in an Activity Away from School for which MCPS Transportation is Not Provided, MCPS Form 560-31 —Needs to be completed before tryouts.
