Girls Basketball

The basketball interest meeting is November 13th at CAT in Mr. Unger's room; tryouts for the girls will begin November 18. For all those interested in trying out but do not have a physical on file in the school, consider getting a physical before the interest meeting at a walk in clinic if there is no time to set up an appointment with the regular pediatrician/physician. Everyone can download all fours forms before the interest meeting on the web page. They are the tryout letter, permission form, medical cards, and health inventory. All four documents must be completed in full before any tryout is allowed. All forms are due November 17.

In order to tryout students must have a Health Evaluation (signed by a doctor and parent or have one already on file at CJMS), a parent permission slip (signed by a parent) and a medical card (signed by a parent).  These three forms can be accessed below or obtained in the locker room at CJMS .
