Interscholastic Sports at CJMS

Activity busses will be available each day for students. Please review parent letter.

All students are required to have a parent permission slip, medical card and a health evaluation signed by a doctor(they are good for 2 years). Students CANNOT try out without these forms in the hands of the coaches at the time they tryout.

In order to tryout students must have a Health Evaluation (signed by a doctor and parent or have one already on file at CJMS), a parent permission slip (signed by a parent) and a medical card (signed by a parent). These three forms can be accessed below or obtained in the locker room at CJMS .
