Medication Procedures

  

1. No medication will be administered in school or during school sponsored activities without the parent’s (guardian’s) written authorization and a physician’s statement for each episode of illness and for each medication ordered.

2. The medication, properly labeled by a pharmacist, must be hand-delivered to the school by the parent. Under no circumstances will either the health department or the school personnel administer medication brought to school by the pupil.

3. The first full day’s dosage of any new prescription will not be given in school.

4. The school must have on file in the pupil’s health folder MCPS Form 525-13 “Authorization To Administer Prescribed Medication.” The parent is responsible for obtaining the physician’s statement, Part II. The nurse shall ensure that all items on the form are completed. A physician may use office stationary/prescription pad in lieu of completing Part II. Information necessary includes: student name, medication name, dosages, and time/s of administration, duration of medication, possible side effects, and physician’s signature and date.

5. No medication will be continued beyond the school year in which it is ordered.

6. The parent is responsible for submitting a new form to the school each time there is a change of dosage or time of administration, as well as for providing medication for as long as it is prescribed.

7. All medication kept in the school will be stored in a secure area accessible only to authorized administering personnel. The school assumes no responsibility for possible loss of the medication.

8. One week after expiration of the physician’s order, the parent must personally collect any unused portion of the medication. Medication not claimed by the parent within one week of expiration of the physician’s order will be destroyed.

9. The Montgomery County Health Department and the Montgomery County Public Schools do not assume responsibility for non-medically prescribed medication administered by the pupil himself/herself.

10. In no case may any health department or school staff member administer any medication, even aspirin, outside the framework of the procedures outlined above and MCPS Regulation 525-13 “Administering Prescribed Medication To An MCPS Pupil While in School.”
