Download the forms, fill them out, and take them to the
Counseling Office. Please do not send the completed forms
electronically. They cannot be processed.

Recommendation Checklist
This checklist is completed by your teacher and used by your

Request for transcripts
Transcript requests should be submitted a month before
the college deadline. You will need to fill out the BLUE
"Request for
Transcript" form
(which is not blue online but you can still use it to submit
your request) for each college you wish the transcripts
to be sent. Additional forms may be needed, especially if
this is your first request. It is important that you follow
the steps regarding transcript requests listed on the "Counseling
Department College Application Process" webpage

Secondary School Report Form
Schools will take the NACAC
form
(National Association of Collage Admission Counseling),
even common application schools. Some schools have their
own forms, but will still take the NACAC
form
.
Where more than one type of form is accepted, it is up to
the student which they prefer. Fill out only the top of
the form (section I).

The Common Application
The Common Application,
http://www.commonapp.org/
,
is the recommended form of 230 colleges and universities
for admission to their undergraduate programs. Many of these
institutions use the form exclusively. All give equal consideration
to the Common Application and the college's own form.
Students you can use Common App Online to complete forms
and submit them electronically or in hardcopy. Or they can
download the forms in PDF and complete them either onscreen
or manually, then print and mail hardcopy.
