Web Regulation - Consent & Release Form

 
  On September 11, 2006 the superintendent approved new Regulation KBA-RB, Educational Purposes and Administration of the MCPS Web. This new web regulation provides a framework for the structure and content of the Montgomery County Public Schools (MCPS) Web.
As part of the new Regulation, the Request to Withhold Directory Information form is used by parents to opt out of having their child's picture or student work on the MCPS web. This replaces any previous forms used by individual schools or offices for this purpose.
  1. Student Privacy Policy and links to forms in all languages.
  2. Request to Withhold Directory Information (101K PDF) This form is somtimes referred to as the FERPA form. This is the form in English for the 2007-08 School Year. The form is available in 5 other languages from Student Privacy Policy. (FERPA stands for the Family Educational Rights and Privacy Act of 1974 and can be referenced in 99.7 of 34 CFP part 99. This act establishes privacy rights for students.)
