TRANSITION SERVICES
NEXT STEPS: SCHOOL TO ADULT LIVING TRANSITION WORKSHOPS

Four School-to-Adult Living Transition workshops will be offered to students and parents this fall. Programs will address eligibility services available in the community and in the schools, student and parent roles and responsibilities in accessing those eligibility services, and tips to promote a successful transition from MCPS to adult living.  The target audience for each workshop will be families of students with Individualized Education Programs (IEP) ages 14-21, or younger as appropriate, with the designated topic of the evening indicated as an outcome in the Transition Plan. Meeting details are below.

I. Adult Services

Description:  Representatives from state and county agencies and the school system will present information about the Maryland Transitioning Youth Initiative for eligible students with significant disabilities. Local adult agencies will give information regarding the services they provide to eligible graduates.

II. Montgomery College Night

Description:  Representatives from Montgomery College will provide general information as well as present an overview of Disability Support Services, Non-Credit Programs and Credit Programs.

PLACE: Earle B. Wood Middle School

III. Postsecondary Education Night

Description: School system representatives will share planning tools and resources to assist students transitioning from school to postsecondary education. Session topics will include: "Thrive in College", "Postsecondary education Options for Students with Intellectual Disabilities" and "Making it on Your Own: Independence!"

IV. Special Topics Night

Description: Participants will attend breakout sessions of their choice, such as, "An Introduction to Transition Services in MCPS from Middle School to High School and Beyond", "Guardianship and Health Care Planning" and "Social Security Income: SSI Information Session."
