The Employee Benefit Plan Summary also includes all forms necessary to change or enroll in benefits. It is available in print format at all health fairs and at ERSC. All eligible MCPS active employees will have an opportunity to meet with benefit program vendors and ERSC staff to discuss open enrollment options on the following dates and times:

Flexible Spending Accounts
Flexible spending accounts offer a great way to save money on healthcare and dependent care expenses. If you are currently participating in an FSA for 2009, then you already know the benefits of using this program. In order to continue your FSA participation in 2010, or to begin contributing to an FSA in 2010, you must take action and select your 2010 contribution amounts during this year’s open enrollment.

Make Changes or Enroll

• MCPS Form 455-20: Employee Benefit Plan Enrollment
  Use this form to enroll in or make change to your benefits.
  
• ERSC Form 300: Calendar Year 2010 Flexible Spending Account Election Form Use this form to set your 2010 contribution amounts to your medial and/or dependent care FSA.
  
• ERSC Form 401: Optional Employee Term Life Insurance Enrollment/Cancellation Form
  Use this form to cancel or enroll in optional employee term life insurance, or to change the beneficiaries of your optional employee term life insurance.
  
• ERSC Form 514: Optional Dependent Term Life Insurance Enrollment/Cancellation Form
  Use this form to enroll your dependents in optional dependent term life insurance.
