Last Minute Reminders
Before leaving the
school for the outdoor education center, please check to make sure the
following things have been done.

1. Parental permission form (345-7) for students and high school student
assistants are with the teacher-in-charge and given to health technician/or
nurse prior to hike-in.

2. Medical Authorization forms is completed for any student with prescription
medication. Medicine is in the original container.
_____
3. Prescription medicine should be packed for delivery to the nurse/health
technician. Prescription medicines, Medical Authorization forms, and Parental
Permission forms are to bedelivered by the first teacher to arrive at
the site.

4. Students should be dressed appropriately for the hike-in. Medicine
for bee stings and asthma is with student(s) who may need them.
_____
5. Each student, high school student assistant, and other staff member(s)
have a lunch.
_____
6. High school student assistants have work permits.
_____
7. Two alphabetized lists (one for boys; one for girls) are available
to give to the nurse/health technician upon arrival)..
_____
8. The bus route is clearly understood by each bus driver.
