Instructions for Sumitting Form 311-10: Request for Interpreters

To submit Form 311-10: Request for Interpreters, follow one of the following sets of instructions. Different MCPS computers have different images, so different processes may work better in different environments.

Process I

  • Download Form 311-10: Request for Interpreters from the MCPS website to a harddisk or server.
  • Navigate to the program Adobe Acrobat 5.0, Adobe Acrobat 6.0, or Adobe Acrobat 7.0. (Adobe Acrobat Reader does not have the capability of saving changes).
  • Open the program.
  • Go to the File menu, select Open, and navigate to the form, opening it from within Adobe Acrobat.
  • Fill out the form and save it with a unique name, preferrably one easily related to the name of the document for which the form is being submitted.
  • This should allow you to save the completed form, with all of the relevant information, and include the form with the document to translated in your e-mail to Interpretations@mcpsmd.org.

Process II

  • Fill Form 311-10: Request for Interpreters on line.
  • Go to the File menu.
  • Select Print.
  • In the Name field, select Adobe PDF.
  • You should then see a dialog box allowing you to save the file.
  • Give the file a unique name and save it with all of the relevant information.
  • Include the form with the document to be translated in your e-mail t o Interpretations@mcpsmd.org.
