When are verification forms due?

Documentation of all service performed during the summer is due to the SSL coordinator by the Last Friday in September.

Documentation of all service performed during the first semester is due to the SSL coordinator by the Friday before first semester exams begin. (This year January 8, 2010.)

Documentation of all service performed during the second semester is due to the SSL coordinator by the Friday before second semester exams begin. (This year June 4, 2010.)

All forms are in PDF format and require Acrobat to access.
